Now hiring Registered Nurses RN for home health visits and case management!


Provide skilled nursing and case management to home health patients primarily in San Francisco County, San Mateo County, and Santa Clara County. 


Duties & Responsibilities:


  • patient teachings



  • medication administration



  • wound care / dressing changes / wound vacs



  • post-surgical care



  • skilled nursing care / direct patient care



  • patient follow-up



  • OASIS Start of Care (SOC), regular SOC, admits & discharges



  • Additional duties as assigned




Desired experience:


  • We are seeking a Registered Nurse (RN) whose license is active a



  • At least one year of Home health Experience



  • Experience with Home Health nursing and Medicare / OASIS a plus

Description 



The RN Case manager will coordinate the care and service delivery of the patient population across the continuum of illness with a focus on the following:




•         Integrates factors related to quality, efficiency and cost-effectiveness in managing care and makes appropriate referrals.




•         Identify cases with potential for high risk complications and act as an advocate for the individual's health care needs




•         Assure timely and appropriate discharges through assessment of patients' post discharge needs and the provision of services necessary to meet those needs.




•         Assumes a leadership role through coordination of care with physicians, staff, patient/family and outside agencies




•         Identify opportunities for improvement of patient care and facilitate support functions.




•         Excellent collaboration and communication skills




•         Expertise in acute care settings in order to assess and identify medical necessity




•         Federal and state requirements




•         Utilization review or case management background, one year preferred




•         Working knowledge of community and governmental resources









Job Requirements



RN with at least 1-2 years clinical experience in an acute care environment


Current licensure as an RN


1 years minimum experience in Case management preferred 


      Current BLS certification required


     Certification in Case Management a plus


Knowledge of clinical standard of clinical practice in critical care preferred


      Knowledge of and experience with OASIS BILLING


 Excellent communication skills


 Flexibility, ability to work with members of the interdisciplinary team


 Bilingual is a plus!
